Base / Technology / A-Spec / Advance / Sport Hybrid (YD3, 3rd gen, 2014-2020)
Low Risk
The YD3 MDX is one of the most proven luxury 3-row SUVs on the market. The 3.5L J35Y V6 is Honda's most reliable large engine — runs to 250k+ miles with synthetic oil. 9-speed ZF auto had early hesitation issues but is largely resolved in 2018. Sport Hybrid is genuinely fuel-efficient and the hybrid system is proven. One of the most recommended used luxury SUVs in its class.

Top concern
9-speed ZF hesitation (early) / VCM motor mount wear
Powertrain
V6 VCM right-rear motor mount wear
Monitor
The Variable Cylinder Management (VCM) system vibrates the right-rear motor mount at idle in 3-cylinder mode. Worn mount causes interior vibration.
With engine warm and AC on at idle, feel steering wheel and seat for vibration.
Est. cost: $300–$600 motor mountSource: Honda VCM mount pattern
9-speed ZF transmission fluid and behavior
Monitor
2018 MDX 9-speed is substantially improved over 2016–2017. Fluid service at 60k miles is recommended despite "lifetime" designation.
Test all driving scenarios.
Est. cost: $400–$600 fluid serviceSource: ZF 9HP service pattern
Hybrid System (if equipped)
Sport Hybrid IMA battery health
Monitor
MDX Sport Hybrid uses a 1.3 kWh IMA battery. Degradation is modest but affects EV assist.
Test fuel economy — should hit 25+ mpg combined.
Est. cost: $2,000–$4,000 IMA batterySource: Hybrid wear pattern

AcuraLink / infotainment
Low Risk
AcuraLink is generally reliable. Test all functions.
Test CarPlay, navigation, and all three zone HVAC.
Est. cost: $0 updateSource: Acura campaigns

SH-AWD rear differential fluid
Monitor
SH-AWD rear differential requires Honda DPS-F fluid every 30k miles.
Test highway cornering for vibration.
Est. cost: $150–$300 serviceSource: Acura SH-AWD service standard

What the VIN tells you
The 3rd-gen MDX (YD3, 2014–2020) came with two completely different powertrains in 2018, and the inspection priorities differ significantly between them. 3.5L J35Y V6 (290hp, 9-speed automatic, FWD or SH-AWD) is the mainstream engine — fitted to Standard / Technology / Advance / A-Spec trims. Known issues: the ZF 9HP 9-speed transmission had documented shift-quality issues from 2014–2016 that improved meaningfully by 2018, and the SH-AWD rear differential needs fluid service every 30k miles (not on Acura's stock interval). Sport Hybrid (Advance trim only, ~5% of 2018 production): 3.0L J30A5 V6 paired with three electric motors and a 7-speed DCT, total system output 321hp. The hybrid has its own concerns — the lithium-ion battery pack, the electric Super-Handling AWD rear motors, and DCT clutch wear are all separate inspection priorities not relevant to the V6. Tell them apart fastest by the badge on the tailgate ("Sport Hybrid" is explicit). Full VIN decode at autovetting.com/inspect.
