26V-507
Low-beam headlights exceed FMVSS 108 maximum output — headlights replaced
2017-2023 Model 3 and 2020-2023 Model Y, 20,349 units. The low beams may be brighter than FMVSS 108 permits, reducing visibility for oncoming drivers. The remedy is replacement of the headlights, not a software change. NHTSA received the report 2026-08-05 and owner letters were mailed 2026-10-03, so on any car you look at today this one is almost certainly still open — check it by VIN rather than asking whether the car is 'up to date'.
21V-912
Front and rear suspension knuckles may fracture
2020-2022 Model Y, 826 units. A knuckle can fracture and let the suspension links separate, which takes away stability and control. Tesla Service inspects and replaces the knuckles free of charge. 826 units is the narrowest population of any campaign on this page, so the model year tells you nothing — only the VIN does, and this is the one item here that can fail structurally rather than annoy you.
23V-385
Steering-wheel fastener may be loose — wheel can separate from the column
2022-2023 Model Y, 137 units. A loose retaining fastener can let the steering wheel come off the steering column while driving. Tesla Service inspects and replaces the fastener; Tesla's own number is SB-23-32-001, letters mailed 2023-07-28. 137 units out of a model year in the hundreds of thousands — unguessable from anything but the VIN, and worth the lookup for exactly that reason.
23V-130
Second-row seat-back frame bolts may be loose
2022-2023 Model Y, 3,470 units. Bolts securing the second-row seat-back frames may not have been tightened at the factory, which degrades how the belt system performs in a crash. Tesla Service inspects and tightens them; Tesla number SB-23-13-001, letters expected 2023-04-25. A free fix, but a crashworthiness item on a family car, so confirm it by VIN rather than by feel.
22V-037
Full Self-Driving (Beta) 'rolling stop' — OTA
2016-2022 Model S/X, 2017-2022 Model 3, 2020-2022 Model Y, 53,822 units. The FSD Beta rolling-stop function could take the car through an all-way stop without stopping. The remedy was an over-the-air update that disables the feature. No dealer visit, so nothing about this appears in a service history.
22V-045
Seat-belt chime may not sound for an unbelted driver (FMVSS 208) — OTA
2021-2022 Model S/X, 2017-2022 Model 3, 2020-2022 Model Y, 817,143 units. The warning chime could fail to sound at startup when the driver is unbelted. Over-the-air remedy, letters mailed 2022-04-01. Read this one together with 24V-376 below: the same FMVSS 208 warning-system defect was recalled again two years after this software fix.
22V-050
Heat-pump valve can reduce defrosting performance (FMVSS 103) — OTA
2021-2022 Model 3/S/X and 2020-2022 Model Y, 26,681 units. A software error could open a heat-pump valve and trap refrigerant, cutting windshield defrost performance. Over-the-air remedy, letters mailed 2022-04-01. Worth knowing in Phoenix for the opposite reason: the same heat-pump hardware is what the cabin depends on, so a car that defrosts poorly in winter is worth a closer look at the pump.
22V-063
Boombox can obscure Pedestrian Warning System sounds (FMVSS 141) — OTA, superseded
2020-2022 Model S/X/Y and 2017-2022 Model 3, 578,607 units. Playing sounds through the external speaker could mask the pedestrian warning tone a quiet EV is required to emit. Remedied over the air — and then superseded by 22V-235, which re-covers the same cars with a wider remedy. A VIN that reads 'completed' against this number was still in scope for the next one.
22V-235
Boombox / Pedestrian Warning System, second attempt (FMVSS 141) — OTA
2020-2022 Model Y/X/S and 2017-2022 Model 3, 594,894 units. Supersedes 22V-063 in NHTSA's own words. The new remedy disables Boombox in Drive, Neutral and Reverse and extends to Summon and Smart Summon, which the first fix missed. Two campaigns, three months apart, same defect — the first remedy did not cover the whole failure.
22V-296
Infotainment CPU overheats while fast-charging — rearview image lost — OTA
2021-2022 Model S/X and 2022 Model 3/Y, 129,960 units. The infotainment processor can lag or restart during or after a fast charge, blanking the centre screen — which on this car is also the rearview camera display, the gear indicator and the warning lights. Over-the-air remedy improving CPU temperature management. Relevant to any car that has lived on Superchargers, which in Phoenix is most of them.
22V-702
Automatic window reversal may not detect an obstruction (FMVSS 118) — OTA
2017-2022 Model 3, 2020-2022 Model Y, 2021-2022 Model S/X, 1,096,762 units. A closing window could apply too much force before reversing. Over-the-air remedy. The largest-population campaign on this page by a wide margin, and one of the cheapest to verify — it is a line in the VIN lookup, not a test you can run on the lot.
22V-844
Taillights may intermittently fail to illuminate — OTA
2023 Model 3 and 2020-2023 Model Y, 321,628 units. One or both taillights can intermittently go dark. Over-the-air remedy, Tesla number SB-22-00-016. Test both taillights during the walk-around anyway: the software remedy addresses the cause NHTSA identified, not a failed lamp assembly.
23V-085
FSD Beta behaviour at intersections and speed limits — OTA
2016-2023 Model S/X/3/Y running or slated to receive FSD Beta, 362,758 units. The software could act unsafely through intersections and respond inadequately to speed-limit changes. Over-the-air remedy, letters mailed 2023-04-15. Ask whether FSD was ever purchased or trialled on the car — the campaign reaches the vehicle regardless, but it tells you how the car was driven.
23V-838
Autosteer safeguards insufficient to prevent driver misuse — OTA
2012-2023 Model S, 2016-2023 Model X, 2017-2023 Model 3, 2020-2023 Model Y, 2,031,220 units. Autosteer's controls may not be enough to keep a driver engaged with this SAE Level 2 system. Over-the-air remedy, letters mailed February 2024. Two million cars, one software push, and no service record on any of them.
24V-051
Brake, Park and ABS warning lights use the wrong font size (FMVSS 105/135) — OTA
Model S/X/3/Y across many years plus the 2024 Cybertruck, 2,193,869 units. The warning-light text was smaller than the standard requires, making critical instrument-panel information hard to read. Over-the-air remedy, Tesla number SB-24-00-003. A reminder that on this car a 'recall' may be a typeface — which is exactly why counting campaigns is a poor proxy for condition.
24V-153
Factory reset muted the Pedestrian Warning System (FMVSS 141) — OTA
2013-2022 Model S/X/3/Y, 6,557 units. A factory reset could leave the pedestrian warning sounds muted, so a pedestrian gets no audible cue. Over-the-air remedy via release 2023.44.30.14, letters mailed 2024-04-27. This is the third FMVSS 141 campaign on this page after 22V-063 and 22V-235 — same requirement, three separate failures, all fixed over the air.
24V-376
Seat-belt warning light and chime may not activate (FMVSS 208) — OTA
2012-2024 Model S, 2015-2024 Model X, 2017-2023 Model 3, 2020-2023 Model Y, 125,233 units. The belt warning light and chime may fail to activate for an unbelted driver. Over-the-air remedy, Tesla number SB-24-00-008. This is 22V-045 again, two years and one software remedy later — the clearest evidence on this page that an over-the-air fix closes a campaign without necessarily ending the defect.
24V-554
Hood latch may not detect an unlatched hood — OTA
2021-2024 Model 3/S/X and 2020-2024 Model Y, 1,849,638 units. After the hood is opened the latch system may fail to detect that it is unlatched, and the hood can then open fully and block the driver's view. Over-the-air remedy, letters mailed 2024-09-24, Tesla number SB-24-00-012. Open and close the frunk during the inspection and watch for the open-hood warning.
24V-935
TPMS warning light may not stay illuminated (FMVSS 138) — OTA
2024 Cybertruck, 2017-2025 Model 3, 2020-2025 Model Y, 696,281 units. The tyre-pressure warning light may not remain lit across drive cycles, so a genuinely underinflated tyre stops announcing itself. Over-the-air remedy, letters mailed 2025-02-15, Tesla number SB-24-00-018. Check cold pressures by gauge rather than trusting the dash on an unremedied car.
26V-283
Rearview camera image delayed in reverse on software 2026.8.6 (FMVSS 111) — OTA
2017 and 2020-2023 Model Y, 2021-2023 Model 3/S, and Model X running software version 2026.8.6, 218,868 units. The rearview image can be delayed when the car is shifted into reverse. Remedied over the air. Note the record's own inconsistency: the over-the-air flag reads false while the remedy text describes an over-the-air update — recorded as the API states it, not reconciled.
